Anything that can go wrong, will... I think horses tend to follow this rule of thumb....

Animals in general will always find trouble. I was working at the dairy barn recently and a cow stuck her head between 2 metal bars that seperates a pen from the main cow area. We could not get her head out no matter what we did and she was suffocating herself! Finally we cut the bar and freed her. I don't know how she fit her head through in the first place.
